Intellectual Disability Case Management

Developmental Services Case Management/Support Coordination coordinates the delivery of services for persons with intellectual/developmental (ID) disability in Henrico, New Kent, and Charles City counties. This includes a comprehensive assessment of individual needs in the areas of housing, day activity/employment, physical health, income/finance, psychological/psychiatric treatment, and others. The assessment is a compilation of developmental, educational, vocational, social, and psychological evaluations as well as information from the person’s family and other service providers.

Once the individual’s needs have been identified, a plan is developed by the individual and his/her team using a person-centered planning process. This process includes the strategies for meeting the needs of the individual and obtaining available services of their choice. The Case Manager/Support Coordinator then assists the individual and family by linking or connecting them with the identified, available service provider(s) or resources.

Once the plan has been implemented, the Case Manager/Support Coordinator monitors the plan on a regular basis to assure that the services address individual health and safety needs, are effective and that the individual is satisfied with the providers and the services available. The Case Manager/Support Coordinator also conducts on-going assessments and plans are adjusted as needed. The service plan itself is updated on a regular basis to reflect changing needs.

Without case management, planning for the needs of individuals with intellectual/developmental disabilities, accessing resources, coordinating services and monitoring progress must be performed by the family in isolation. Individuals with intellectual/developmental disabilities are productive contributing members of their communities. Many individuals with intellectual/developmental disabilities receive a wide range of services that assist with ensuring health and safety as well as assist, when desired, the individual to engage in community living. Case Managers/Support Coordinators assess need, identify, and access resources and monitor service delivery to ensure it meets the individual needs and reflects their choices.

Henrico Area Mental Health & Developmental Services targets day services, individual supported employment, and residential services to individuals with intellectual/developmental disability who are age 18 and above. The Developmental Services Case Management/Support Coordination Unit serves all ages and is a long-term service.

Individuals who have a diagnosis of intellectual/developmental (ID) disability have impaired comprehension ability. The degree of impairment is usually referred to as mild, moderate, severe, or profound. The American Association on Intellectual and Developmental Disabilities defines intellectual/developmental disability as significant limitations in both intellectual functioning and adaptive behavior originating before the age of 18. IQ tests measure intellectual functioning, and a variety of standardized tests are used to measure adaptive behavior such as conceptual, social, and practical skills.

Respite

Respite is a support service that is available to families who have a family member with intellectual/developmental disability. Limited funding is available to enable income eligible families to purchase respite care from a variety of providers.

Client Assistance

Limited funds are available for individuals to help with expenses such as behavior consultation, summer school, special equipment, clothing, and emergencies.
