Request Inmate Visitation-West

Jail West Visitation Request Form
Review the information below prior to completing this form.
- Based on availability, reservations can be made no more than seven (7) days in advance and no less than forty-eight (48) hours in advance, Monday – Friday 8:00am to 4:30pm.
- All inmates receiving in person visits must be vaccinated.
- Eligible inmates can receive one (1) thirty minute in-person visit per month.
- All visitors must provide proof of vaccination by attaching a copy of a valid identification card and vaccination record to this form in advance AND provide the original documents for review upon arriving for visit.
- Visitors will be limited to two (2) persons. For safety reasons, children under the age of twelve are not permitted.
- You will receive a confirmation email once all criteria have been verified.
- Only registered visitors will be allowed in the facility on the scheduled visitation day. (Only 2 per visit).
- All registered visitors must adhere to the current mask mandate when visiting inmates.
